The CCC leads in care coordination, triage, and innovative approaches within a complex environment. Qualifications include an active RN license in the U.S., graduation from an approved nursing program (BSN preferred), and eligibility under VA standards.

Qualifications

• Licensed registered nurse in the United States with active license.
• Graduate of an approved nursing program with current licensure.
• Foreign degree equivalency not applicable for VA standards.

Responsabilités

• Provide clinical triage and care coordination for community care.
