Job summary

Community Care Physicians is seeking an experienced Nurse Practitioner or Physician Assistant to join Vascular Health Partners in the Capital Region, New York. This role supports a premier vascular surgery practice with four board-certified surgeons and a dedicated APP team.

The position involves outpatient care from a state-of-the-art interventional suite in Latham, with travel to satellite offices in Saratoga Springs, Queensbury, and Amsterdam. Expect a dynamic, clinically focused environment.

Qualifications

• Current New York State licensure as a NP or PA.
• Graduate of an accredited NP or PA program.
• DEA registration (or ability to obtain).
• Strong clinical assessment and communication skills.
• Ability to work independently while collaborating closely with physicians.

Responsibilities

• Evaluate, diagnose, and manage patients with arterial and venous disease in the outpatient setting
• Perform comprehensive vascular assessments and develop treatment plans in collaboration with vascular surgeons
• Manage patients with conditions including peripheral arterial disease (PAD), carotid artery disease, aneurysms, chronic venous insufficiency, varicose veins, DVT, and peripheral vascular disease
• Provide pre-operative and post-operative evaluations and follow-up care
• Review vascular ultrasound and diagnostic testing results
• Coordinate care with referring primary care physicians and specialists
• Educate patients regarding vascular disease prevention, treatment options, and lifestyle modifications
