Community Care Physicians, in partnership with Vascular Health Partners, seeks an experienced Nurse Practitioner or Physician Assistant to join a premier vascular surgery practice in the Capital Region.

The role involves outpatient evaluation and management, collaboration with four board-certified vascular surgeons, and travel to satellite offices in Saratoga Springs, Queensbury, and Amsterdam to provide outpatient vascular care.

Formación

• Current New York State licensure as a Nurse Practitioner or Physician Assistant.
• Graduate of an accredited NP or PA program.
• DEA registration (or ability to obtain).
• Strong clinical assessment and communication skills.
• Ability to work independently while collaborating with physicians.

Responsabilidades

• Evaluate, diagnose, and manage patients with arterial and venous disease in the outpatient setting.
• Perform comprehensive vascular assessments and develop treatment plans with vascular surgeons.
• Manage conditions such as PAD, carotid disease, aneurysms, chronic venous insufficiency, DVT, and PVD.
• Provide pre-operative and post-operative evaluations and follow-up care.
• Review vascular ultrasound and diagnostic testing results.
• Coordinate care with referring physicians and specialists.
• Educate patients on vascular disease prevention, treatment options, and lifestyle changes.
