Job summary

Humana Inc. invites a Primary Care Physician to join CenterWell Senior Primary Care in The Villages, FL. This onsite role focuses on outpatient medicine, preventive care, and health maintenance for seniors in a unique care model.

Reporting to the Associate Medical Director, you will maintain patient histories and coordinate care across a multidisciplinary team. The position emphasizes patient-facing clinical work with a 40-hour week; candidates should be board-certified or eligible in Geriatric,

Qualifications

• MD or DO; board certified or board eligible in Geriatric Medicine, Family Medicine or Internal Medicine.
• Valid, unrestricted medical license in the state of employment (or willingness to obtain).
• Current, unrestricted DEA license.

Responsibilities

• Provide outpatient primary care focusing on prevention, health maintenance and disease prevention.
• Maintain patient medical history and records; coordinate referrals to specialists.
• Refer patients to specialists and support continuity of care across the care team.
• Impact clinic strategy and participate in TB screening program as part of Humana/Senior Bridge initiative.
• Schedule: Monday–Friday, 8:00am–5:00pm; see 12–14 patients per day.
