• Spanish-speaking strongly preferred for patient population.

Responsibilities

• Provide high-touch primary care with longer visits and continuity of care.
• Manage chronic diseases in seniors within a value-based care model.
• Collaborate with an established team including physicians, NPs, MA, scribe, and RN to support patient care.
