Job summary

Millennium Physician Group, LLC is hiring a physician to provide comprehensive outpatient primary care for adults, focusing on preventive care and chronic disease management. You will collaborate with interdisciplinary teams to improve outcomes and patient experience within a value-based care model.

The role requires board certification/eligibility in Family or Internal Medicine, an active Florida license (or ability to obtain), and a DEA license (or ability to obtain).

Qualifications

• Board certification or eligibility in Family Medicine or Internal Medicine.
• Active Florida medical license or ability to obtain one.
• DEA license or ability to obtain prior to start.
• Passion for value-based, patient-centered care.
• Experience with Medicare Advantage and population health preferred.
• Bilingual in English and Spanish is a plus.
• ACN-certified physicians encouraged to apply.
• Experience with AthenaOne EMR is a plus.

Responsibilities

• Provide comprehensive outpatient primary care for adult patients.
• Develop long-term relationships focused on preventive care and chronic disease management.
• Collaborate with interdisciplinary care teams to improve quality outcomes and patient experience.
• Utilize evidence-based medicine to deliver value-based care.
• Participate in quality initiatives that improve patient health while reducing unnecessary healthcare utilization.
