Job summary

Renown Health is seeking a Director of Value Based Care to drive transformation across value-based programs and optimize clinical pathways. You will oversee major initiatives, align clinical practice to cost and quality measures, and collaborate with health plan partners.

The role requires deep knowledge of value-based care, EMR optimization, and strong leadership to manage multidisciplinary teams and deliver measurable outcomes while maintaining a patient-centered focus.

Qualifications

Bachelor's degree or higher in Healthcare, Business Administration, Public Administration, or related field.
MBA, MHA, or MPH preferred.
Minimum five (5) years of leadership experience in healthcare.
Minimum five (5) years of healthcare tech and data management experience.
Strong understanding of value-based care and industry trends.
Deep understanding of EMR optimization as a driver for value-based care.

Responsibilities

Develop and lead the Value-Based Care Model and clinical best practices.
Oversee daily clinical and business operations of departments.
Manage value-based programs: CHAs, Outreach, Wellness, Primary Care at Home, CCM, TCM.
Drive clinical and cost performance indicators and program alignment to outcomes.
Develop budgets and monitor dashboards for clinician performance and quality metrics.
Collaborate with clinical leaders, partners, and data teams to integrate analytics.
Establish a culture of service excellence and monitor patient satisfaction.
