Job summary

CVS Health in Jamaica, NY is seeking a Registered Nurse to join our clinical care team within a Value-Based Care model. You’ll focus on patient health outcomes and long-term wellness, supporting assessment and triage for walk-ins and calls.

Responsibilities include protocol-driven chronic condition management, wound care, med administration, patient education, and medication reviews. You’ll work with providers to optimize care plans and ensure safe transitions.

Qualifications

• Active, Unrestricted RN licensure in the resident state.
• 2+ years healthcare experience; RN preferred.
• BLS or CPR certification required.
• EMR experience required.

• Support patient assessment and prioritization for walk-in and telephonic triage.
• Execute protocol-driven chronic condition management.
• Perform skilled nursing procedures including wound care and medication administration.
• Provide education to patients and caregivers on chronic conditions.
• Conduct medication reviews within nurse-led visits.
• Onsite testing and use of patient data to identify gaps in preventive screenings.
• Review and act upon high-priority abnormal lab results.
• Coordinate with providers to manage complex care plans and transitions.
• Utilize protocols for medication management and renewals.
• Delegate tasks to medical assistants and phlebotomists as needed.

Requirements

Education:

• Active, Unrestricted Registered Nurse Licensure in the state of residence

Experience:

• 2+ years of healthcare experience, working as an RN preferred

Licenses & Certifications:
