Honeycomb ManagementGroup is seeking a Value-Based Care Coordinator (LPN) to facilitate operations and communications for value-based care agreements. Reporting to the Quality and Stars Program Manager, you will coordinate planning, oversight, and project completion to ensure performance in contracts and compliance.

You will promote process improvement, involvement in solution development, and knowledge of care coordination and preventive services.

Qualifications

• Graduate of an accredited Practical Nursing Program.
• Current Licensed Practical Nurse (LPN) license in practicing state.
• Three (3) years of experience as an LPN preferred.
• Must possess and maintain a valid driver’s license.
• Experience in HEDIS and STAR Measures are preferred.

Responsibilities

• Demonstrate knowledge of Value-Based policies and procedures through training.
• Provide superior patient service with effective communication.
• Analyze and solve complex problems with attention to detail.
• Support quality initiatives and outreach for wellness scheduling and recalls.
• Use Motivational Interviewing to engage patients and align health goals.
• Meet deadlines and adapt to changing priorities.
• Maintain strong EMR skills and navigate patient engagement software.
• Assist and provide clinical support at other locations as needed.
