Job summary

Compassus seeks a Care Liaison Manager with registered nurse leadership experience to drive value-based care coordination across home health and hospice services. You will lead market strategy, engage hospital executives, and align operations with JV goals in a dynamic healthcare environment.

The role requires a BSN, plus potential MSW or related clinical home health background, and at least 3 years in healthcare project management or program leadership.

Qualifications

• Bachelor of Science in Nursing required; Master’s in Social Work with clinical home health experience considered.
• 3+ years of complex healthcare business project management experience required.
• Prior success managing healthcare teams, business development, or program management preferred.

Responsibilities

• Lead implementation of care coordination activities for Hospice and Home Health services within geographic area.
• Engage hospital and JV leaders to promote value-based care initiatives.
• Collaborate with HCC leaders to align operations with VBE goals and market strategy.
• Manage and train clinical liaison and CTA staff; ensure cross-department collaboration.
