You'll apply continuous process improvement, use critical thinking, and promote engagement in solution development while bringing knowledge of care coordination, preventive health, and screenings to the role.

Qualifications

• Knowledge of Value-Based policies and procedures.
• Experience with care coordination concepts.
• Ability to navigate patient engagement software.
• Demonstrated attention to detail and problem-solving.

Responsibilities

• Research and follow Value-Based policies; attend training webinars.
• Provide superior patient service through effective communication.
• Develop solutions for complex problems with strong analytics.
• Interpret complex policies and regulate compliance.
• Facilitate outreach, schedule wellness appointments, support quality.
• Use Motivational Interviewing to engage patients.
• Meet deadlines and adapt to changing priorities.
• Maintain strong EMR skills.
• Navigate patient engagement software for recalls.
• Understand chronic conditions related to gap closures.
• Perform basic Excel tasks.
• Provide clinical support at other site locations.
• Perform other duties as assigned.
