Job summary

You will perform comprehensive medication management reviews, assess therapeutic alternatives, and support transitions of care and adherence initiatives in care management teams. Responsibilities include collaborating on ACO quality metrics, documenting encounters in Epic, and aligning workflows to quality benchmarks.

Qualifications

• PharmD degree required; licensed or eligible to practice in MA, NH, ME, RI.
• Board certification (BCPS, BCACP, or BCGP) highly recommended.
• Minimum of three (3) years of related clinical practice experience, ambulatory care/health system preferred.

Responsibilities

• Provide direct patient care and pharmacotherapy within high‑risk patient populations.
• Perform Comprehensive Medication Management reviews and transitions of care.
• Support ACO quality metrics and care management initiatives to improve outcomes.
• Document encounters and communicate with providers via Epic and EHRs.
