Position Overview

Care Advantage Inc is seeking a dedicated Value Based Care Clinical Lead Registered Nurse to join our team, contributing to our mission of delivering exceptional homecare services across the Mid-Atlantic area (VA, MD, DE, DC).

Responsibilities

• Oversee VBC relationships and data, ensuring quality service and cooperation across all provided services.
• Collaborate with senior leadership to organize and analyze program performance data.
• Communicate with payer partners and prepare reports as needed.
• Design and deliver educational materials for patients and caregivers to promote positive outcomes.
• Lead VBC Clinical Rounding sessions, documenting discussions and outcomes.
• Monitor clinical service quality and update workflows and policies periodically.
• Engage with patients, families, and staff to improve overall satisfaction and outcomes.
• Track and communicate trends, ensuring contractual performance measures are met or exceeded.
• Set and achieve quarterly goals.

Qualifications

• Current Registered Nurse license (Multi-State Compact preferred).
• Minimum 5 years of nursing experience; VBC experience in home healthcare preferred.
• Bachelor's degree in Nursing or related field preferred.
• Leadership, management, and marketing skills required.
• Proficiency in Microsoft Office Suite and Teams.
