Qualifications

  • Minimum of 2 years of direct patient care experience.
  • Managed Care, IPA, or Health Plan experience.
  • Experience conducting wellness or comprehensive visits virtually or in-home.

  • Conduct assessments, including comprehensive annual wellness exams in patients' homes.
  • Counsels and educates patients and families about available benefits.
  • Documents clinical information and coordinates care with providers.

Job description

Monogram Health is looking for skilled Nurse Practitioners and Physician Assistants eager for the opportunity to make a difference in patients' lives. The Advanced Practitioner at Monogram Health is a key member of an integrated Care Team which includes a Registered Nurse and a Social Worker. The patients we serve often struggle with multiple serious diseases. Our Nurse Practitioners and Physician Assistants help patients improve their quality of life in the home and slow the progression of kidney disease, enabling positive health outcomes.

Your Impact

Using your skills in this position will allow you to deliver personalized compassionate medical care to individuals mainly with CKD and/or ESRD/ESKD. You will also be responsible for caring for patients, maintaining accurate and current patient records and scheduling, and administering follow-up appointments to patients as required. Your gifts as a healthcare professional are urgently needed. In healthcare systems, the patient has too often become secondary due to processes and incentives that don’t positively impact the patient for the long term. Roles and Responsibilities

  • Conducts assessments, including comprehensive annual wellness exams in patients' homes and virtual environments
  • Counsels and educates patients and families about available benefits and programs to promote healthier lives
  • Documents clinical information such as chief complaint, diagnosis, medical, family, and social history, review of systems, examinations, medications, allergies, assessment, and plan
  • Coordinates care with primary care providers, specialists, and ancillary services
  • Completes all documentation and paperwork promptly
  • Maintains quality of care standards as defined by the practice
  • This position is remote within the employed state and may require periodic training or meetings outside that state
  • Delivers evidence-based, timely care to reduce hospitalizations, maximize quality of life, and prioritize patient health and satisfaction
  • Prescribes medications, orders tests, and collaborates with Monogram physicians
  • Monitors and reports patient outcomes to measure effectiveness
  • Educates patients about preventive monitoring and schedules diagnostic testing with physicians
  • Assists with patient enrollment in educational programs
  • Participates in care team meetings
  • Maintains knowledge of disease diagnosis and prevention
  • Assesses patient health status and implements care plans
  • Follows up and evaluates patient progress
  • Performs other duties as assigned

Position Requirements

  • Bilingual (English/Spanish) highly preferred
  • Basic Life Support (BLS) certification required; company supports certification process
  • Active and unrestricted RN and NP or PA licenses
  • Board certification (NP: ANCC/AANP; PA: NCCPA)
  • Current DEA certificate
  • Ability to work autonomously without direct supervision
  • Valid driver’s license, access to transportation, and insurance
  • Proficiency with medical instruments, equipment, and data management systems
  • Effective verbal and written communication skills with various stakeholders
  • Understanding of federal and state regulations including HIPAA and OSHA
  • Minimum of 2 years of direct patient care experience
  • Managed Care, IPA, or Health Plan experience
  • Experience conducting wellness or comprehensive visits virtually or in-home
