Job summary

Allegheny Health Network is seeking a Population Health Program Manager to collaborate with CIN practices and stakeholders to optimize clinical integration and value-based reimbursement. You will train practices, analyze performance, and help set regional goals with leaders and care teams.

The role focuses on improving patient outcomes while containing costs, with emphasis on adoption of optimized workflows, panel management, and care path programs across the CIN region.

Qualifications

• Requires a Bachelor’s degree in a health-related field and at least 5 years of population health management experience.
• Experience with CINs, VBR programs, or pay-for-value initiatives is preferred.
• Strong data analysis skills and ability to develop action plans from performance reports.

Responsibilities

• Provide training to assigned practices/institutes to utilize CIN cost/utilization reports and analytics tools.
• Analyze performance data to identify opportunities and develop practice-specific action plans.
• Collaborate with regional operations and LAC leaders to define goals and report progress.
