Dignity Health - Mercy General Hospital in Sacramento, CA is seeking a Circulatory Device Coordinator RN to oversee VAD patient care across the continuum, from inpatient to home settings. The role emphasizes clinical leadership, program development, and collaboration with the Advanced Heart Failure team.

You will design VAD clinic services, lead performance improvement initiatives, and ensure meticulous documentation and quality standards while maintaining strong patient and family engagement.

Qualifications

• BSN degree required or equivalent experience.
• 5+ years of professional clinical experience in critical care.
• 1 year of experience caring for Ventricular Assist Device patients.
• Registered Nurse: CA license.
• BLS through AHA.
• ACLS through AHA.

Responsabilités

• Oversee care of high-risk VAD patients across the continuum (inpatient, home, clinics, facilities) in collaboration with the Medical Director.
• Design and implement VAD Clinic services and improvement programs; act as clinical liaison to physicians, staff and families.
• Provide 24/7 availability to support the clinical management team.
• Coordinate referral, screening, testing requirements, consults and procedures; educate staff and physicians on VAD care.
• Develop performance improvement activities to improve VAD patient outcomes.
• Build therapeutic relationships with patients and families and connect with community resources.
