Qualifications

• MD or DO with board certification in respective specialty.
• Current licensure in any US state or territory.

Responsibilities

• Independently manages a panel of patients with acute and chronic conditions with emphasis on prevention.
• Develops and implements care plans to achieve population health goals.
• Collaborates with nurses and pharmacists as part of the care team.
• Orders and interprets labs and imaging; prescribes interventions.
• Provides patient and family education and coordinates referrals.
