Job summary

Freedom Home Care is seeking a dependable RN to perform VA-authorized assessments, reassessments, and care planning for veteran clients across Cincinnati and surrounding areas. You will schedule visits, travel to client homes, and maintain compliant documentation in a salaried full-time role.

The position emphasizes self-direction, professional communication, and teamwork with the VA and care coordination staff to support veterans in their homes.

Qualifications

• Active RN license in the State of Ohio is required.
• Experience with home or VA-authorized care is preferred but not required.
• Ability to manage a flexible full-time schedule and travel to client homes.

Responsibilities

• Complete initial in-home assessments for veterans starting services.
• Perform VA reassessments for ongoing clients.
• Create and update care plans based on assessments and VA guidelines.
• Educate caregivers on care plans and updates.
• Document assessments, care plans, and recommendations promptly.
• Communicate findings with the care coordination team.
• Coordinate follow-up appointments with VA staff as needed.
