Job summary

Avail Health is seeking an experienced Mobile Registered Nurse to deliver in-person clinical assessments for Medicare Advantage I-SNP members in long-term care facilities within the Cincinnati service area. The RN will conduct HRAs, document findings, and help develop ICPs, while collaborating with NPs and facility teams.

This 1099 PRN role requires independent practice, strong assessment and communication skills, and willingness to travel between facilities.

Qualifications

• Current, active, unrestricted Ohio RN license or compact license.
• Minimum of 3 years of nursing experience with older adults or chronic/complex conditions.
• Strong clinical assessment and documentation skills.
• Ability to work independently with professional judgment within RN scope.
• Strong interpersonal and communication skills with patients, ARs, staff.

Responsibilities

• Independently conduct in-person HRAs for I-SNP members in long-term care facilities.
• Complete comprehensive assessments across medical, functional, cognitive, and behavioral domains.
• Document assessment findings to support ICP development and updates.
• Engage members and ARs to align care planning with preferences and goals.
• Coordinate with facility staff and interdisciplinary care teams as appropriate.
• Identify changes in condition or care gaps and communicate escalation needs.
• Complete HRA documentation accurately within required timeframes.
• Partner with NPs to support CDAs and facilitate virtual NP encounters.
