Job summary

A Place at Home in Albuquerque, NM is seeking a dedicated Registered Nurse to join our team. This on-site role focuses on delivering compassionate, client-centered home care to VA clients and coordinating care with our staff.

You will manage care plans and records, uphold policies, coordinate with the VA, and advocate for clients while maintaining professional relationships with families and healthcare partners in the Albuquerque area.

Qualifications

• Registered Nurse license required and CEUs maintained.
• 1–2 years experience in home care preferred but not required.
• Demonstrable knowledge of healthcare processes, problem solving, critical thinking, and time management.
• Excellent organizational, leadership, and communication abilities.
• Articulate, efficient, self-starter with professional client relationships.

Responsibilities

• Advocate for clients as mandatory reporter.
• Develop and update care plans and log them in the ClearCare system.
• Conduct periodic care plan reviews and coordinate between VA and A Place At Home- Albuquerque.
• Support caregivers and provide guidance via phone.
• As RN Advocate, assemble PHRs and coordinate healthcare across settings.
