Job summary

A Place at Home Albuquerque/Santa Fe is seeking a dedicated Registered Nurse to join our home-based senior care team. The RN will manage VA Clients in line with agency policies and Veterans Administration guidance, delivering compassionate, patient-centered care in clients' homes and through coordinated care plans.

Responsibilities include developing and updating care plans, logging accurately in the Clear Care system, conducting periodic reviews, and serving as a resource for caregivers.

Qualifications

• Must be a Registered Nurse with maintained licensure requirements and CEUs.
• 1 to 2 years’ experience, preferably in the Home Care Industry (not mandatory).
• Demonstrable knowledge of healthcare processes, proactive problem-solving, critical thinking, and time management skills.
• Excellent organizational, leadership, and communication abilities.
• Articulate, efficient, self-starter, able to maintain professional client relationships.

Responsibilities

• Advocate for clients as mandatory reporter.
• Develop and update care plans, ensuring they are correctly logged into the Clear Care system.
• Conduct periodic client care plan reviews and manage all communication between the VA and A Place At Home- Albuquerque
• Act as a resource to caregivers, providing support via phone when needed.
• Act as an RN Advocate, assembling Personal Health Records (PHRs), coordinating healthcare, and advocating for clients in various healthcare settings.
