  • Ensures clinical documentation is accurate, complete, and timely per guidelines
  • Maintains licensure, CPR certification, and CEU requirements

Follows guidelines for universal precautions in accordance with accepted professional practices.* ***Interdisciplinary Group*** + Attends, participates, and may facilitate interdisciplinary group conferences [IDG] and special care conferences. + Assumes responsibility for establishing and maintaining effective working relationships with team members. + Identifies specific patient/caregiver needs and make referral to appropriate team members. + Assists the administrative assistant with managing the referral process, provider assignments, provider.* ***Documentation*** + Ensures accurate, complete, succinct, and timely clinical documentation in accordance with VIA Health Partners guidelines and the requirements of licensure and certification standards. + Reviews and updates pain contracts for each palliative care provider on a weekly basis.* ***Professional and Personal Accountability*** + Demonstrates competency in knowledge and skills. + Ensures that VIA Health Partners professional reputation is maintained and projected. + Assumes responsibility for professional development and staying abreast of current trends in the healthcare field. Incorporates new information and methods into practice. + Maintains current professional licensure, CPR certification and CEU requirements. + Maintain productive and professional relations with external agencies and healthcare providers. + Exhibits time management skills. Able to plan workday while remaining able to respond appropriately to crisis.* ***May perform other duties as required.***## Qualifications**Minimum Qualifications*** A Diploma or associate degree in nursing is required; bachelor’s degree preferred.* A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN) is required.* The Nurse Licensure Compact (NLC) allows a registered nurse (RN) and licensed practical/vocational nurse (LPN/LVN) to have one multistate license in a primary state of residency (the home state) and to practice in other compact states (remote states), while subject to each state’s practice laws and discipline.* Current CPR required; CHPN preferred.* Minimum 2-year related experience. Experience in direct care of patients with life limiting illnesses required. Related experience may include hospice and palliative care; family medicine; internal medicine; geriatrics, oncology; home health care; medical surgical.* Computer proficiency is required
