Job summary

Fairview Health Services seeks a Registered Nurse (RN) Care Coordinator for the Clinics and Surgery Center (CSC) on the University of Minnesota East Bank campus in Minneapolis. This full-time 1.0 FTE role focuses on a Urology specialty, coordinating care and educating patients, families and staff to ensure safe, efficient transitions along the health continuum.

Responsibilities include care planning, referrals, patient education, medication management and liaison duties to promote

Qualifications

• 3 years specialty experience in care coordination.
• 5 years licensed RN with clinical experience and ability to learn new specialty area.
• MN Registered Nurse License.
• Basic Life Support (BLS) certification.

Responsibilities

• Accesses, analyzes, plans and provides follow-up care.
• Facilitates timely referrals and fosters positive relations with community providers.
• Evaluates and documents status and follows patient progress through phone calls, test results and report preparation.
