Fairview Health Services in Minneapolis, MN is seeking an experienced Registered Nurse Care Coordinator for the Clinics and Surgery Center on the University of Minnesota East Bank campus, with travel to St. Paul.

The role is 1.0 FTE, salaried, with clinic hours Mon-Fri 8:00 am - 5:00 pm, and partial remote work. You will coordinate care, provide disease management, and act as liaison between patients and medical staff.

Qualifications

• 3 years of specialty experience in care coordination.
• 5 years licensed RN with clinical experience in a new specialty.
• MN RN License.
• BLS certification.

Responsibilities

• Accesses, analyzes, plans and provides follow-up care.
• Coordinates care across transitions and services.
• Facilitates timely referrals and maintains provider relationships.
• Educates patients and families on treatment plans.
• Documents patient progress and evaluates care quality.
• Triages concerns and responds under physician direction.
• Keeps up to date with clinical guidelines and best practices.
• Participates in research and data collection as needed.
