Fairview Health Services in Minneapolis, MN is seeking an experienced Registered Nurse Care Coordinator for the Clinics and Surgery Center on the University of Minnesota East Bank campus, with travel to St. Paul.
The role is 1.0 FTE, salaried, with clinic hours Mon-Fri 8:00 am - 5:00 pm, and partial remote work. You will coordinate care, provide disease management, and act as liaison between patients and medical staff.
Fairview has an immediate opening for a Registered Nurse (RN) Care Coordinator at the Clinics and Surgery Center (CSC) located on the University of Minnesota East Bank campus in Minneapolis, Minnesota and will travel to St. Johns Hospital in St. Paul. This position is approved for 1.0 FTE (80 hours per pay period / two-week pay period) and is a salaried position. Clinic operating hours are Monday - Friday 8:00 am - 5:00 pm. This position is partially remote. This position focuses on an area of a specific medical specialty or chronic care condition (i.e. endoscopy, enterostomal therapy, mental health, total parenteral nutrition, bone marrow transplant). Serves as a resource and educator to patient, medical and hospital staff within the specialty. Coordinates care efforts for a safe, effective, efficient and patient centered transition along the health continuum. Provides pro-active disease management using evidence based practices. Acts as a liaison between patient/family, medical staff and others involved in treatment, promoting patient centered care based on jointly determined health care goals and priorities.

The posted pay range is for a 40-hour workweek (1.0 FTE). The actual rate of pay offered within this range may depend on several factors, such as FTE, skills, knowledge, relevant education, experience, and market conditions. Additionally, our organization values pay equity and considers the internal equity of our team when making any offer. Hiring at the maximum of the range is not typical.
