Job summary

Sweetwater Care is seeking an experienced LVN for leadership within our California facility in Parlier. The role emphasizes nursing management in long-term care, ensuring patient care aligns with physician orders, policies, and guidelines.

Responsibilities include QA participation, infection control adherence, patient and family education, and timely medication/treatment administration. Prior PCC experience is preferred, with strong teamwork and communication required.

Qualifications

• Currently licensed LVN in California.
• One-year nursing management experience preferred, not required.
• Long-term care experience required.
• PointClickCare charting experience required.

• Demonstrates knowledge and appropriate use of Infection Control Manual.
• Participates in Quality Improvement Program.
• Utilizes Quality Life Manual for patient placement and monitoring.
• Notifies other departments of patient changes ( room changes, LOA, new admits, discharge).
• Conducts family and patient education.
• Administers medications timely according to facility policy.
• Administers treatments timely according to facility policy.
• Receives, transcribes, and carries out physician orders when allowed by Nurse Practice Act.
• Makes rounds with physicians when necessary.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.

Qualifications
