Halcyon Home in Waco, TX is hiring a Hospice LMSW for a full-time position to support our geriatric hospice population in the area. You will conduct hospice social work visits, assess social and emotional needs, coordinate care with the interdisciplinary team, document progress in Homecare Homebase EMR, and offer counseling to patients and families facing end‑of‑life challenges.

Minimum one year of hospice and social work experience; LMSW license required; competitive pay and wonderful team

Responsibilities:

• Perform hospice social work visits in assisted living, independent living communities, skilled nursing facilities, and residences for hospice patients within Houston area.
• Conduct comprehensive assessments of patients' social and emotional needs
• Assist with coordination of care and access to community resources
• Maintain detailed and accurate documentation of patient interactions and progress
• Utilize Homecare Homebase EMR (knowledge of which is a plus)
• Develop personalized care plans in collaboration with the interdisciplinary team
• Provide counseling and support to patients and families facing end-of-life challenges

Requirements

• Licensed as an LMSW
• Knowledge or experience utilizing Homecare Homebase EMR would be a plus
• Minimum of one year of hospice care experience
• Minimum of one year of social work experience
• Compassionate and empathetic approach to working with the elderly
• Strong clinical and assessment skills
• Excellent communication and interpersonal abilities
• Ability to work independently and manage time effectively
