Job summary

Halcyon Home in Dallas, TX is hiring a Hospice LMSW full time to support our geriatric hospice population. You will perform hospice social work visits in various settings, assess social and emotional needs, and coordinate care with the interdisciplinary team.

You will document thoroughly in Homecare Homebase EMR, develop personalized care plans, provide counseling to patients and families, and help connect them with community resources. A current LMSW license and hospice experience are required.

Qualifications

• Licensed LMSW in good standing.
• Minimum of one year hospice care experience.
• Minimum of one year of social work experience.
• Strong communication and interpersonal skills.
• Ability to work independently and manage time effectively.

Responsibilities

• Perform hospice social work visits in various settings (assisted living, skilled nursing facilities, residences).
• Conduct comprehensive assessments of patients' social and emotional needs.
• Coordinate care and access to community resources.
• Maintain detailed and accurate documentation of patient interactions and progress.
• Utilize Homecare Homebase EMR (knowledge a plus).
• Develop personalized care plans with the interdisciplinary team.
• Provide counseling and support to patients and families facing end-of-life challenges.

Responsibilities:

• Perform hospice social work visits in assisted living, independent living communities, skilled nursing facilities, and residences for hospice patients within Houston area.
• Conduct comprehensive assessments of patients' social and emotional needs
• Assist with coordination of care and access to community resources
• Maintain detailed and accurate documentation of patient interactions and progress
• Utilize Homecare Homebase EMR (knowledge of which is a plus)
• Develop personalized care plans in collaboration with the interdisciplinary team
• Provide counseling and support to patients and families facing end-of-life challenges

Requirements

• Licensed as an LMSW
• Knowledge or experience utilizing Homecare Homebase EMR would be a plus
• Minimum of one year of hospice care experience
• Minimum of one year of social work experience
• Compassionate and empathetic approach to working with the elderly
• Strong clinical and assessment skills
• Excellent communication and interpersonal abilities
