Responsibilities:

• Perform hospice social work visits in assisted living, independent living communities, skilled nursing facilities, and residences for hospice patients within Houston area.
• Conduct comprehensive assessments of patients' social and emotional needs.
• Assist with coordination of care and access to community resources.
• Maintain detailed and accurate documentation of patient interactions and progress.
• Utilize Homecare Homebase EMR (knowledge of which is a plus).
• Develop personalized care plans in collaboration with the interdisciplinary team.
• Provide counseling and support to patients and families facing end-of-life challenges.

Requirements

• Licensed as an LMSW.
• Knowledge or experience utilizing Homecare Homebase EMR would be a plus.
• Minimum of one year of hospice care experience.
• Minimum of one year of social work experience.
• Compassionate and empathetic approach to working with the elderly.
• Strong clinical and assessment skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently and manage time effectively.
