Position Title: Urgent Care Registered Nurse

Department: Nursing

Supervisor: Director of Nursing

Status/Hours Per Week: Non-exempt / 40 hours

Primary Function

The UC Nurse provides quality nursing care and participates as part of a member of the Urgent Care team.

Duties & Responsibilities

  • Provide initial assessment of patient presenting to Walk in/Urgent Care in order of medical priority and time of arrival.
  • Elicit a chief complaint and perform a clinical assessment of the complaint and determine the relative urgency of each patient.
  • Document all nursing assessments and interventions using SOAP note format. Triage note should include: triage time; patient’s name, age, or date of birth; allergies; past medical history; current medications; subjective and objective observations; vital signs; nursing interventions; and patient response to interventions.
  • Initiate appropriate nursing interventions such as first aid measures, ice, bandaging, wound irrigations, BLS techniques, administering oxygen, eye irrigation, twelve lead EKG, and/or obtain specimens, etc.
  • Upon the order of a physician, nurse practitioner, or physician assistant administer medications, injections, intravenous fluid and perform other procedures including but not limited to: ear irrigation, debridement of small surface area burns, and/or application of splints, etc.
  • Triage patient telephone calls according to telephone triage guidelines.
  • Handle calls for managed care approvals as needed.
  • Facilitate care according to the Urgent Care protocols, policies and procedures including that for Fast Track, Fever Protocol, Diversion Protocol and managing patients in acute medical distress, patients with high fever, and trouble breathing etc., and ensuring provider access whether in Urgent Care or from other providers in Primary Care or Women’s Health.
  • Communicate and facilitate hospital admission information to appropriate provider and maintain documentation.
  • Facilitate access to Urgent Care for hospital discharges that are being followed up on at the health center if without access to PCPs appointment slots.
  • Provide follow-up to patients seen in walk-in as appropriate (e.g. picking up prescriptions, progress of recovery/medication adherence etc.).
  • Assist providers with examinations, treatments, specimen collection, and/or surgical procedures as needed.
  • Communicate with providers about laboratory results of diagnostic tests as appropriate.
  • Conduct nursing visits.
  • Check DAILY or MONTHLY per review schedule and record or report irregularities for the UC fridge, Emergency Code Cart and Medication Closet.
  • Complete required forms as relevant (e.g. for medical transport, emergency code blue etc.).
  • Maintain patient flow through the department.
  • Participate in supporting providers in reporting all diseases cited in the Massachusetts Department of Public Health list of Reportable Diseases to the State and Local Board of Health when applicable.
  • Provide patient education as needed.
  • Participate in UCC emergency (code blue) protocol.
  • Maintain clinic licensing standards for compliance in areas for nursing (Emergency Code Cart, Mediation Closet, Sample Meds etc.).
  • Participate in department and health center meetings.
  • Maintain current knowledge of developments in nursing field and be responsible for self-development through continuing education opportunities.
  • Perform other related duties as required.
  • Provides cross-coverage in primary care nursing and OB/GYN nursing as needed.

Minimum Basic Knowledge

Licensed Registered Nurse in state of Massachusetts, (B.S.N. preferred).

Experience & Qualifications

  • Two years of nursing experience, preferably in urgent care
  • Experience in a setting handling patients with a diverse set of needs.
  • Bilingual language skills in Spanish or Portuguese Creole preferred.
  • Ability to communicate clearly to patients and to staff.
  • Ability to work well with others, including patients and staff.
  • Critical attention to detail and ability to follow specific protocols.
  • Desire to work within a community health environment ideal.

Independent Action

Functions independently within scope of nursing practice and health center policies and goals, while referring specific problems to supervisor when clarification of operating policies or procedures is required or needed.

Define Access Level to PHI: Level 4: Authorized to access full health information. UCC position and job responsibilities involve the provision of patient care and working as a clinical team member. Staff in this category level, although allowed full access, should only access the necessary information for each respective treatment encounter/circumstance.
