The Anticoagulation Management Services at MGH is looking for a dynamic Staff Nurse to join their team! You will work with the collaborating physician to enhance anticoagulant care across the continuum. Use your experience and clinical expertise as a pivotal partner with patients, nurses, physicians, case managers, and pharmacists to facilitate implementation of an anticoagulation plan of care across clinical services.

As a Staff Nurse, your daily assignment will include monitoring warfarin patients' INR values, adjusting medication to maintain a prescribed range, and providing patient education, care and support during one-on-one virtual visits and over the telephone. As the AMS transforms to manage patients on all direct oral anticoagulants the ideal candidate will have the courage to explore the unexplored, have a fresh outlook, capacity to reframe problems and a burning desire to get things done. Remote position so an ideal candidate must be able to work independently.

Job Summary

Accountable for interpreting the plan of medical care, assessment of patients' clinical decision-making regarding nursing care, assuring nursing care is provided in a safe and competent manner, providing individualized nursing care, and evaluating nursing care for groups of patients.

Essential Functions
-Maintain accurate, detailed reports, and records.
-Administer medications to patients and monitor patients for reactions or side effects.
-Record patients' medical information and vital signs.
-Monitor, record, and report symptoms or changes in patients' conditions.
-Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate patient care plans.
-Modify patient treatment plans as indicated by patients' responses and conditions.

Associate's Degree Nursing required or Bachelor's Degree Nursing preferred

- Knowledge of medical terminology.
- Excellent verbal and communication skills.
- Ability to accurately screen and triage acute patients.
