Job summary

Prevea Health is seeking an Advanced Practice Provider (NP or PA) to support Urgent Care in the Lakeshore Region. This PRN position focuses on assessment, treatment, and prevention under physician supervision, with emphasis on coordination of care and patient education.

The role involves 1–3 years of clinical experience, ongoing education, and collaboration with a multi-disciplinary team to deliver high-quality care in an urgent care setting.

Qualifications

• Graduate of a master's level nurse practitioner program OR graduate from an accredited physician assistant program.
• 1–3 years clinical experience preferred.

Responsibilities

• Provides assessment, evaluation, and management to patients with minor, acute, chronic, & episodic illness & injuries.
• Collaborates with patients to review health background and plan future health actions.
• Provides case management and referrals as needed.
• Communicates test results and follow-up instructions to patients and families.
• Orders appropriate diagnostic tests and medications as needed.
• Maintains patient records in the EMR with accurate documentation.
• Collaborates with physicians to develop and implement patient care plans.
