Job summary

Local MD is seeking an Advanced Practice Provider (APP) in Palatine, IL, for full-time, part-time, or per diem roles. The APP delivers high-quality care, performs comprehensive assessments, diagnoses and manages treatment plans, and collaborates with the Medical Director and clinic staff.

Responsibilities include patient education, EMR documentation, and participation in quality improvement. Excellent collaboration and flexibility across shifts are expected.

Qualifications

• Active Illinois APRN license (NP) or Illinois PA license
• Active RN license (NPs only)
• National certification (AANP/ANCC for NPs; NCCPA for PAs)
• BLS certification
• DOT/FMCSA Certified Medical Examiner

Responsibilities

• Conduct initial patient assessments (history & physical) and establish baseline health profiles
• Diagnose and treat acute and chronic conditions; prescribe medications
• Coordinate referrals and collaborate with specialists as needed
• Educate patients and families on self-management and treatment plans
• Document accurately in the EMR per Local MD standards and ensure HIPAA compliance
• Provide clinical leadership and training to staff; participate in quality improvement initiatives

Education

Master’s Degree or Doctorate in Nursing Practice / PA program
