Job summary

The APP will be responsible for implementing effective patient management strategies, and providing education to empower patients. Certification as a Nurse Practitioner or Physician Assistant in Indiana is required along with associated responsibilities to uphold care standards.

Qualifications

• Active Indiana APRN License (NPs) or Indiana PA License.
• National Certification (AANP/ANCC for NPs; NCCPA for PAs).
• Master’s Degree or Doctor of Nursing Practice (NP track).
• Basic Life Support (BLS) certification.

Responsibilities

• Perform initial patient assessments including history-taking and physical examinations.
• Diagnoses and treats acute and chronic health conditions.
• Educates patients and family members on self-management of illnesses.
• Prepares accurate and thorough documentation in the EMR.
