Job summary

Floyd Valley Healthcare in Le Mars, IA is seeking a Nurse Practitioner or Physician Assistant for our Urgent Care Clinic on a part-time, PRN basis. The role emphasizes primary care within family practice, care coordination, and education, with weekend rotation.

Responsibilities include providing continuous patient care, coordinating referrals, and collaborating with the care team. Weekend Urgent Care is open Saturdays 8a–12p and Sundays 1p–5p, with additional PRN coverage during winter months.

Qualifications

• Master’s Degree in Nursing required.
• Licensed in the state of Iowa with a minimum of one year of experience.
• ACLS/BLS certification preferred; certification required as an employee.
• Mandatory Reporter training certification preferred; required as an employee.

• Provide primary health care in family practice, acting as care provider and coordinator, educator, and consultant.
• Ensure continuity of care, coordinate referrals, and education for patients.

The nurse practitioner functions to provide continuity of care, coordinates referrals, and ensures comprehensive health care of patients according to their needs through prevention, treatment, symptom management and education.* Saturday Weekend rotation every 8th weekend + PRN for Saturday and Sunday Urgent Care to add additional providers during winter months.
