Job summary

PASCO (Personal Assistance Services Of Colorado) is hiring an RN Case Manager to coordinate and oversee clinical care for clients in the Denver Metro service area. You will conduct nursing assessments, develop care plans, and collaborate with physicians, caregivers, and the team to ensure safe, compliant care.

The role requires Colorado RN licensure, a nursing degree, and CPR/First Aid certification, with preference for home health experience.

Qualifications

  • 1 year RN experience required (acute care or medical/surgical preferred).
  • Home Health nursing experience preferred.
  • Current license to practice as a Registered Nurse in Colorado.
  • Bachelor's/Associates/Diploma in Nursing.
  • Current CPR / First Aid certification.

Responsibilities

  • Coordinate and manage comprehensive care for an assigned caseload.
  • Complete nursing assessments and develop, implement, and update Plans of Care.
  • Collaborate with physicians, caregivers, clients, and interdisciplinary team members.
  • Interpret physician orders and monitor client progress, and adjust care plans as needed.
  • Perform skilled nursing visits per physician orders and nursing standards.
  • Monitor client outcomes and intervene as needed.
  • Review lab results and communicate findings to providers.
  • Supervise and mentor LPNs, CNAs, and other caregivers.
  • Ensure documentation accuracy and regulatory compliance.

What You'll Do

As an RN Case Manager, you'll oversee the clinical care of an assigned caseload while ensuring services are delivered safely, effectively, and in compliance with state and federal regulations.

Key Responsibilities
  • Coordinate and manage comprehensive care for an assigned caseload of clients.
  • Complete nursing assessments and develop, implement, and update individualized Plans of Care.
  • Collaborate with physicians, caregivers, clients, and interdisciplinary team members to ensure quality, coordinated care.
  • Interpret physician orders and monitor client progress, making clinical recommendations and care plan adjustments as needed.
  • Perform skilled nursing visits in accordance with physician orders and accepted nursing standards.
  • Monitor client outcomes, identify changes in condition, and intervene appropriately.
  • Review laboratory results and communicate significant findings to appropriate providers.
  • Prioritize and schedule client visits to meet clinical needs and documentation deadlines.
  • Supervise and mentor LPNs, CNAs, and other caregivers, ensuring competency, quality of care, and regulatory compliance.
  • Complete supervisory visits, performance feedback, and required competency documentation.
  • Investigate concerns, respond to clinical issues, and support caregivers with complex situations.
  • Educate clients, families, and caregivers on health conditions, treatments, and available community resources.
  • Maintain accurate, timely, and compliant clinical documentation, including admissions, recertifications, transfers, and discharges.
  • Partner with the Quality Assurance team to ensure documentation accuracy and regulatory compliance.
  • Attend nursing meetings as scheduled.
  • Maintain HIPAA compliance and protect the confidentiality of all client information.
Requirements

What We're Looking For

Experience
  • 1 year experience practicing as an RN required, experience in acute care, medical / surgical nursing strongly preferred.
  • Experience and skill set matching assignments.
  • Home Health nursing experience preferred.
Education And/or Licenses
  • Current license to practice as a Registered Nurse in the state of Colorado.
  • A Bachelors in Nursing, Associates in Nursing or Diploma in Nursing required.
  • Current CPR / First Aid certification.
Knowledge, Skills Or Abilities
  • Clinical Judgment: Ability to independently collect and interpret data about a consumer, arrive at a conclusion and identify the appropriate action. This involves problem solving, decision making and critical thinking rooted in clinical knowledge and experience.
  • Consumer Service: Ability to establish and maintain relationships with consumers, and provide professional appropriate, compassionate and empathetic care to consumers.
  • Maintaining Commitments: Ability to consistently fulfill commitments, show up when promised, and maintain strong communication.
  • Collaboration: Ability to maintain a positive attitude, demonstrate flexibility and balance self-interest with the interests of the agency.
  • Knowledge of State, Federal and Licensing rules and regulations for care delivery and documentation by Home Care Agencies.
  • Knowledge of unique issues related to care delivery for people with a variety of disabilities.
  • Knowledge of therapeutic action, side effects and interaction of medications.
  • Knowledge of the principles of growth and development over the life span.
  • Ability to assess data reflective of the consumer’s status.
  • Ability to communicate effectively both orally and in writing.
  • Ability to operate a computer, familiarity with Microsoft products, specifically Outlook, and ability to receive and process work in an Electronic Health Record system.
  • Ability to work with a diverse population.
