Job summary

Mary Washington Healthcare in Fredericksburg, VA is seeking an Unit Clinical Care Coordinator (Unit C3) to lead patient progression, DRG and TLOS assignments, and discharge planning to improve throughput. You will collaborate with emergency and surgical services, CDI, UR, and care teams to develop care plans and remove barriers, guiding timely care and safe discharge.

Qualifications include RN with BSN preferred, 3+ years acute care, 2+ years critical care, and strong communication with crisis

Qualifications

• Minimum of three (3) years recent acute care nursing experience.
• Bachelor of Science in Nursing is preferred; RN license in Virginia or reciprocal compact state.
• Minimum two (2) years of critical care experience.
• Able to work independently, managing time, multiple priorities, and resources to achieve goals.
• Able to maintain a calm demeanor and command during time of crisis management.

Responsibilities

• Coordinates patient care activities with C3s to ensure appropriate level of care and patient flow at the unit level.
• Identifies and removes unit barriers that impede patient flow.
• Assesses clinical presentation against MCG to determine admission status, DRG and LOS; validates with CDI as needed.
• Assesses patients in observation/outpatient status for status change; progresses care as indicated.
• Coordinates with UR Nurse to align payor and status requirements with clinical progression.
