Job summary

Montage Health in California is seeking an experienced Care Coordinator on Unit C3 who will drive patient progression and discharge activities in collaboration with the interdisciplinary team. Qualifications

• Minimum 5 years of acute hospital experience preferred.
• Bachelor’s degree in nursing is preferred.
• Strong communication and organizational skills are essential.

Responsibilities

• Facilitate patient progression and discharge planning with Care Coordinators.
• Collaborate with Hub to ensure appropriate patient placement and level of care.
• Evaluate discharge needs upon admission and ensure a complete plan.
• Lead SNAP rounds with physicians to discuss progression and barriers.
• Communicate post-acute plan of care to patients and families.
• Identify discharge opportunities and update patient status.
• Coordinate with UM and unit staff to reflect status in Care Progression systems.
