Job summary

Sapphire Ridge Health and Rehabilitation, part of the Ascent Healthcare Management family, seeks an LPN Unit Manager to oversee resident care delivery, develop care plans, and administer nursing services.

You will work closely with the Director of Nursing, participate in process improvements, ensure accurate patient documentation, and manage staffing and discharge processes to maintain high-quality resident care.

Qualifications

• Licensed Nurse with current state license.
• 1–3 years related experience; supervisory experience preferred.
• Current CPR certification.
• Strong documentation skills and clinical proficiency.
• Excellent verbal and written English communication.

Responsibilities

• Participate in process improvement initiatives to improve customer experience, workflow, and work environment.
• Collect information and assist in completing evaluations of residents at admission and discharge.
• Notify resident or legal representative of pending discharge 24 hours in advance.
• On discharge date, chart complete discharge summary with planning and referrals.
• Maintain complete and accurate sign-in/out records for staffing hours.
• Collaborate with the Director of Nursing to resolve grievances with staff, families, and residents.
• Address staffing shortages by arranging for replacement staff.
• Participate in infection control and resident care programs and offer recommendations.

Position Description:

The LPN Unit Manager is responsible for oversight of the delivery of care to residents. Accommodate resident needs, develop care plans, administer nursing care.

What you will do:

• Participate in process improvement initiatives that improve the customer experience, enhance work flow, and/or improve the work environment.
• Collect information and assist to complete evaluations of residents at admission and discharge.
• Notify resident or legal representative of pending discharge twenty four (24) hours in advance.
• On date of discharge, chart complete discharge summary, to include discharge planning and referrals.
• Keep complete and accurate record of sign in/out sheets for staffing hours.
• Work closely with the Director of Nursing in resolving grievances with nursing staff, family members, and residents.
• Respond to need for staffing shortages by arranging for replacement staff.
• Participate in the programs related to infection control, exposure control, and resident care to provide proactive improvement. Identify areas of concern and offer recommendations and suggestions to the Director of Nursing.

Other special projects and duties, as assigned.

What you need:

• Licensed Nurse with current state license.
• One (1) to three (3) years related experience; supervisory experience preferred.
• Must have a current/active CPR certification.
• Excellent technical and documentation skills.
• Effective verbal and written English communication skills.
• Demonstrated basic to intermediate skills in Microsoft Word, Excel, Power Point and Outlook, Internet and Intranet navigation.
• Highest level of professionalism with the ability to maintain confidentiality.
• Approachable, flexible and adaptable to change.
• Function independently, and have flexibility, personal integrity, and the ability to work effectively with employees and vendors.
