Job summary

Optalis Healthcare (Health & Rehabilitation Centers) is seeking an Unit Manager to assume 24-hour responsibility for resident care on an assigned unit. The role supervises licensed nurses and direct staff, ensures regulatory compliance, and promotes resident-centered care aligned with facility philosophy and nursing standards.

The position oversees care plans, documentation accuracy, survey readiness, and interdisciplinary collaboration.

Qualifications

• Graduate of an accredited nursing program with current RN or LPN license in Ohio.
• 2+ years of long-term care or skilled nursing experience.

Responsibilities

• Supervise and evaluate nursing staff performance on the assigned unit.
• Ensure care plans are implemented and updated based on resident needs.
• Monitor documentation for accuracy, timeliness, and regulatory compliance.
• Respond to resident/family concerns and ensure dignity and resident rights are respected.
• Support survey readiness and quality assurance activities.
• Coordinate interdisciplinary care and participate in care planning meetings.
• Demonstrate strong clinical assessment and supervisory skills.
• Maintain knowledge of CMS/state regulations and SNF standards.
• Communicate effectively and maintain proper documentation.
• Demonstrate organizational and time management skills.
