Qualificações

• Specialized Diploma graduate from an accredited school of nursing.
• 3 years as a Registered Nurse in an acute care setting.
• Effective communication and partnership with patients and families.
• Patient-and-family-centered practice and cultural awareness.
• Care coordination across multidisciplinary teams.
• Patient safety and risk awareness in clinical practice.
• Knowledge of discharge planning and post-discharge needs.
• Assessment and nursing diagnosis in clinical judgment.
• Health information documentation and medical records.

Responsabilidades

• Interacts with co-workers, visitors, and staff to provide safe, compassionate, coordinated care tailored to patient preferences.
• Communicates with interprofessional teams and serves as a clinical resource on policies and performance improvement.
• Collaborates with medical staff to align interdisciplinary care plans; assess needs and make care recommendations.
• Facilitates unit throughput and discharge planning to optimize patient flow.
• Provides and coordinates staff schedules; resolves scheduling and staffing issues.
• Monitors performance and participates in governance initiatives and clinical ladder activities.
• Supports nursing development and mentorship within the unit and councils.
