Job summary

Weeks Medical Center in Lancaster, NH, seeks a Unit Coordinator/LNA to assist with nursing teams in Medical/Surgical or Emergency Department. Responsibilities include ADL support, patient safety, documentation, and coordination with nurses to ensure quality care.

Qualifications

• High School Diploma or equivalent.
• Maintains NH Nursing Assistant license (not required for unit coordinator).
• Maintains current active BLS certification.
• Prefer 1-2 years of experience.

Responsibilities

• Incorporates appropriate skills in all interactions and delivery of care relative to ADLs, safety and protection, patient care skills, evaluation and equipment use/management.
• Communicates with patients, primary nurse, other patient care team members and management in an accurate, timely manner; reports pertinent information; completes timely charting; protects confidentiality.
• Develops and maintains human care relationships; advocates for patient needs and rights; adapts care for diverse populations.
• Manages self and resources; organizes and prioritizes multitask workload; maintains clean environment; communicates professionally.
• Deliver care with a focus on health promotion; provide information and assess barriers to learning.
• Meets standards related to telephone etiquette, customer service, attendance and confidentiality.
• Answer patient call bells promptly using “How can I help you?”
• Care for multiple patients; support to all patients in facility.
• Observe patient rights.
• Provide personal care and assist with activities of daily living.
• Check patient rooms for linens and bath items; avoid overstocking.
• Oral hygiene twice daily; assist if unable or provide care where appropriate.
• Feeding: pass and pick-up food trays.
• Performs additional duties as assigned.
• Maintain work area clean and orderly.
