Community Hospital of the Monterey Peninsula (CHOMP) seeks a Care Coordination professional to lead patient progression and discharge planning within Unit C3. You will partner with care teams, ensure proper placement, and drive timely discharge while maintaining quality patient experience.

The role emphasizes SNAPP rounds with physicians, collaboration with nurses and social workers, and utilization management to optimize care and resources. CA RN licensure and BLS are required.

Formación

• RN license required in California.
• Minimum 5 years acute care hospital experience preferred.
• Strong communication and coordination skills.
• Knowledge of financial reimbursement and payer requirements preferred.

Responsabilidades

• Acts as spoke of the Hub by partnering with other Care Coordinators to facilitate patient progression and care coordination.
• Collaborates with the Hub to ensure appropriate patient placement and level of care (ICU, Telemetry, Med/Surg).
• Evaluates discharge needs upon admission with a complete discharge plan.
• Works with interdisciplinary team to progress care towards discharge while maintaining quality and patient experience.
• Leads SNAP rounds with the physician to discuss patient progression and barriers to discharge.
• Communicates with patients and families about post-acute plan of care.
• Identifies discharge opportunities and updates patient status and planned discharge time.
• Works with primary nurse and social worker on discharge needs/barriers; initiates UM nurse intervention as needed.
• Ensures Care Progression technology reflects unit and patient status (transfers, discharges, alerts).
• Monitors patients in observation/outpatient status to ensure timely discharge or status change.
• Communicates service event and patient preparedness issues to Order Coordinator.
• Ensures completion of prep prior to testing/procedures and identifies outpatient transition opportunities.
• Assists Clinical Documentation Specialist in engaging physicians to reflect severity of illness in documentation.
• Serves as a consultant and educator with medical staff regarding status, placement, payor requirements, and utilization of resources.
