Job summary

Your role supports administrative workflows, tracks referrals and authorizations, and serves as a consistent point of contact for discharge coordination across care teams and external providers. Per diem hours apply.

Qualifications

• Bachelor's degree preferred in healthcare, social services, human services, or related field.
• Two years of healthcare/case management, discharge planning, or care coordination experience preferred.
• American Heart Association BLS certification required or obtained within policy timeframe.

Responsibilities

• Coordinate discharge planning and transitions for IRU and Westland House patients.
• Gather living situation, transportation, equipment needs, and barriers from patients and families.
• Arrange administrative discharge plans for home health, outpatient therapy, SNF, or other settings.
• Track discharge targets, tasks, referrals, authorizations, and appointments.
• Support transfers and communicate with receiving providers to finalize arrangements.
• Collaborate with nursing, therapy, social services, and physicians to support organized transitions.
