Job summary

UCLA Health is seeking a Utilization Management Supervisor (Non-Clinical) to oversee non-clinical patient navigation services for high-risk and high-utilizer populations. You will supervise coordinators, navigators and concurrent nurses, ensuring efficient workflows and timely access to care.

You will oversee scheduling, transportation coordination, referrals, and post-discharge outreach while collaborating with interdisciplinary teams to address barriers to care and improve outcomes.

Qualifications

• High school diploma, GED or equivalent experience.
• 4+ years of experience in Managed Care Case Management and care coordination – REQUIRED.
• 3+ years of inpatient and ambulatory Managed Care program/referrals – REQUIRED.
• 3+ years in a leadership or supervisory role – REQUIRED.
• Experience with Intensive Case Management, Utilization Management, SNF and Home Health programs.
• Familiarity with Medicare Advantage and value-based care models.
• Experience with electronic health records (e.g., Epic/CareConnect).

Responsibilities

• Supervise and lead Patient Navigators and Coordinators in UM, SNF, ED follow-up, and Home Health workflows.
• Oversee patient navigation activities: scheduling, transportation, DME, and community referrals.
• Monitor work queues, referrals, discharge tracking, and care transitions for timely follow-up.
• Support post-discharge outreach and coordination to ensure adherence to care plans.
• Collaborate with interdisciplinary teams to address barriers to care and engage patients.
• Manage staff performance, scheduling, attendance, payroll/timekeeping, and training.
• Analyze operational data to support quality improvement and compliance.
• Lead process improvements and optimize departmental workflows and procedures.
• Support system-related functions in care coordination and utilization workflows.
