Job summary

HealthPartners North Suburban Family Physicians in Minnesota seeks a casual Triage RN to support three clinic locations—Roseville, Hugo and Lino Lakes. Shifts are Mon–Fri, starting between 7:45am and 8:30am and ending between 4:15pm and 5:00pm.

The role involves triage using EMR protocols, patient assessment, care planning and patient education. Responsibilities include documentation, collaboration with the care team, delegation where appropriate, and acting as a resource for staff to maintain

Qualifications

• Graduate of an accredited school of nursing.
• Current RN licensure in Minnesota; Wisconsin licensure within two months of hire.
• Current BLS certification for health professionals.
• Strongly demonstrated written and oral communication skills.

Responsibilities

• Provide triage support for clinic patients using EMR protocols and standing orders.
• Conduct comprehensive assessments and establish/adjust plans of care.
• Delegate nursing tasks appropriately and supervise nursing activities.
• Create and implement dynamic patient teaching plans based on identified needs.
• Serve as a resource for other staff and coordinate care with the health team.
