What you will accomplish:
  • Plan and conduct intervention opportunity evaluations, respond to urgent alerts and remote patient monitoring alerts as needed to help drive high quality care at a lower cost
  • Recognize clinical scenarios that require escalation to the internal team nurse practitioner
  • Work directly with the member, via various forms of communication, to develop and achieve patient-centered chronic care management goals
  • Develop and update care plans for members, considering caregiver and family support
  • Apply clinical experience and judgment to utilization and care management activities
  • Manage interventions for quality outcomes to reduce avoidable admissions, readmissions, and ED utilization
  • Collaborate with engagement and product teams to promote quality outcomes, optimize service experience, and effective resource use for complex or elevated medical issues
We are seeking a compassionate and driven individual who has:
  • The ability to work full-time on 8, 10, or 12-hour shifts during weekdays and commit to four weekend shifts per month (required)
  • A Compact AND New York nursing license (required)
  • Bilingual fluency in English AND Spanish (preferred)
  • Graduation from an accredited nursing program (required)
  • At least 2 years of nursing experience with adult and geriatric patients (required)
  • Confidence in clinical skills for telephonic triage/assessment (required)
  • Ability to work remotely with a private home workspace (required)
  • A genuine desire to serve others and help those in need
  • High-speed home WiFi/data connection to support company IT equipment

Pay range: $85,000 - $101,000/year, based on experience and location. Actual compensation may vary.
