Overview

Triages incoming calls from patients and/or care givers, assesses needs, provides appropriate clinical recommendations, and guides patients to the appropriate level of care or resources. Communicates and coordinates care when regional centers are closed. Works under general supervision. This role is remote and requires a New York State nursing license. The shift is Wednesday - Sunday, 4:00 p.m. - 12:00 a.m.

Compensation Range

$85,000.00 - $106,300.00 annually

What You Will Do

• Collect and compile medical and social information; assess physical, social, and psychological needs of patients calling after-hours.
• Provide appropriate care advice and direct callers to suitable resources or levels of care, using clinical protocols and guidelines.
• Document all interactions accurately in the computer system, including details and outcomes of calls.
• Coordinate after-hours care by contacting relevant parties to ensure continuity and follow-up.
• Support field staff and collaborate with clinical management on complex issues.
• Assist in orienting new nurses to the contact center role.
• Maintain high clinical knowledge and participate in professional development.
• Participate in on-call coverage and special projects as needed.

Qualifications

Licenses and Certifications:

• Current NY State Registered Nurse license

Education:

• Associate's Degree in Nursing (required)
• Bachelor's Degree in Nursing (preferred)

Work Experience:

• At least two years in Home Care, Ambulatory Care, Emergency Room, or related fields
• Clinical competence and multitasking ability in fast-paced settings
• Excellent communication, analytical, and interpersonal skills
• Proficiency in Microsoft Windows preferred
