Job summary

Native American Rehabilitation Association Northwest (NARA NW) seeks a Triage Registered Nurse for our in-person clinics in Portland, OR. The RN will provide clinical triage, nursing assessments, patient education, and care coordination within outpatient primary care settings.

Qualified applicants have an Oregon RN license (with WA license within six months), a BSN, strong critical-thinking skills, and experience with EHR systems.

Qualifications

• Current, unrestricted RN license in Oregon required.
• Must obtain/maintain WA RN license within 6 months of hire.
• BSN degree required.
• Strong clinical assessment and critical thinking skills.
• Ability to provide patient-centered care and education.
• Proficient with EHR systems and documentation.
• Ability to work independently and with a team.
• Must pass background checks and drug screen.

Responsibilities

• Provide clinical triage via calls, walk-ins, and nurse consultations.
• Coordinate prenatal and anticoagulation case management and tracking.
• Maintain clinical registries and program data per regulations.
• Document assessments and care in the EHR.
• Collaborate with providers, pharmacists, and the care team.
• Assist with ordering and maintaining clinic supplies and meds.
• Participate in Quality Improvement initiatives and agency activities.
• Provide cross-coverage and additional nursing support as needed.
• Support perinatal care program patient education and follow-up.
