The role involves assessing acuity, acting as a liaison between patients and physicians, and delivering education to patients and staff.

This full-time position offers comprehensive benefits, training, and opportunities for professional development, with shifts from 8:30am-5pm Monday, Wednesday, Thursday, Friday

Qualifications

• Graduate of an accredited RN program.
• Current Illinois RN license.
• American Heart Association BLS CPR certification.
• At least 3 years of experience in a hospital or outpatient setting.
• Spanish speaking is a plus.

Responsibilities

• Triage all incoming telephone calls and electronic messages, prioritizing and routing to the provider as appropriate.
• Document all communications in the electronic medical record.
• Schedule patient visits with providers when telephone assessment indicates.
• Coordinate emergency and hospital transfers and related paperwork.
• Administer medications and treatments per Nursing Standards as ordered.
• Provide patient and staff education on clinical issues.
• Support OSHA and CDC guidelines and assist in staff orientation.
